Submitting My Sample

• Download your submission form from this page or from your confirmation email.
  
• Print out and follow the instructions on the form. (Don’t worry if you don’t have a printer; please just include all of the information requested on the form on a piece of paper.)
  
• Follow the instructions on the form and post your sample to our testing facility along with your form.
